Here is part of a field guide written for a sample 504 plan: a sixth grader with type 1 diabetes.
What to Ask For
These are questions and requests you can raise with the 504 coordinator, the school nurse, or in writing at any time. You do not have to wait for the annual review.
- Request a copy of the diabetes care plan. Ask the school nurse or 504 coordinator to share the written diabetes care plan that the 504 plan references. Confirm that the plan reflects what your child's medical team recommends, including blood glucose target ranges and out-of-range response steps.
- Ask how the plan travels to field trips and non-classroom settings. Request a clear statement of who is responsible for accommodations when your child is off campus or in a setting where the regular teacher and nurse are not present.
- Ask what your child should do if an accommodation is not provided. A simple, child-friendly step your child can take, and a parallel step for you, gives the plan real-world traction.
